Facial sagging and double chin: an honest guide to non-surgical solutions

Why the face starts to sag, how to know your degree of laxity and what works in each case: HIFU, radiofrequency or biostimulators. Medical guide.

Dr. Camila Guzmán · Medical Director

Mature woman examining the firmness of her jawline and chin in the mirror

One day you look at a photo and something has changed: the jawline no longer draws the same line, the neck looks different, a shadow appears under the chin that wasn't there before. Facial sagging doesn't arrive overnight — it settles in slowly, which is why it raises so many questions: is it too early to treat it? Do creams work? HIFU or radiofrequency? Will I end up needing surgery?

This guide answers those questions with the same judgement we use in consultation: first understand what is sagging and at what depth, then choose the right tool for that plane — and be honest when something is not going to work. No magic promises: the full ladder of options, from habits to surgery.

Why the face sags (and at what depth it happens)

From your mid-late twenties, the skin gradually loses collagen — classic studies estimate around 1 % per year, and the loss accelerates markedly with menopause. At the same time, facial fat compartments shift and the SMAS — the musculo-aponeurotic layer that supports the face, the same one tightened in a surgical facelift — loses tension. Sun, tobacco and abrupt weight loss speed up the whole process.

Understanding this changes the conversation: sagging is not a single-layer problem, which is why no single treatment solves everything. Each technology works at a specific depth:

Each treatment acts on its own layer

0-2 mm

Skin (epidermis and dermis)

448 kHz radiofrequency
2-4 mm

Subcutaneous fat

HIFU 3.0 mm
4-5 mm

SMAS — the facelift plane

HIFU 4.5 mm
> 5 mm

Muscle

The SMAS is the plane tightened in a surgical facelift, and HIFU is the non-invasive technology that reaches it (3.0-4.5 mm). 448 kHz radiofrequency works higher up, on skin quality and firmness. That is why they don't compete: they complement each other.

What degree of sagging do you have? An orientative self-test

The right treatment depends on the degree. These three descriptions serve as orientation — real confirmation comes from a professional assessment:

1Mild laxity

Skin quality
  • Skin looks less smooth and luminous than before
  • The jawline is still defined, but the skin "responds" more slowly to gestures
  • First fine lines and more visible pores

Your likely path: cosmetics with medical judgement + 448 kHz radiofrequency to stimulate collagen and slow the process.

2Moderate sagging

The contour blurs
  • The jawline starts losing definition in profile photos
  • Early double chin or a "shadow" under the chin
  • Nasolabial folds more marked at the end of the day

Your likely path: HIFU on the SMAS for the tightening effect, with radiofrequency to maintain skin quality.

3Marked sagging

Visible descent
  • Clear descent of the jawline and neck, visible from the front
  • Excess skin that folds or hangs
  • Loss of support and volume in the midface

Your likely path: consider combining with collagen biostimulators — and, if there is clear excess skin, an honest conversation about the surgical option.

The ladder of solutions, explained honestly

There is no universal star treatment: there is a ladder, and the professional way is to start on the step that matches your degree and the affected plane.

1Habits and cosmetics with judgement

The foundation

For everyone, at any age.

Daily sunscreen (photoaging is the main accelerator of sagging), well-prescribed retinoids, not smoking and enough sleep. Honest expectation: it prevents and improves texture, but no cream tightens an already sagging jawline — it doesn't reach that depth.

2448 kHz radiofrequency

Mild laxity · skin quality

For skin losing firmness and glow, and as maintenance after other treatments.

Controlled deep heat that stimulates collagen in the dermis. Comfortable sessions — a pleasant, massage-like warmth —, no social downtime, in protocols of 6-10 sessions. Ideal for slowing the process early and sustaining results over time.

3HIFU 22D-MAX

Moderate sagging · lifting effect

For a blurred jawline, double chin and neck, when the problem is already in the SMAS.

Focused ultrasound that reaches 3.0-4.5 mm — the surgical facelift plane — generating progressive tightening over 2-3 months. One session a year is usually enough. It is the non-invasive reference for lower-third descent.

4Biostimulators and injectables

When support is missing

For faces that, besides tightening, need to recover structure or define the chin and jawline.

Biostimulators (calcium hydroxyapatite, poly-L-lactic acid) induce your own collagen progressively; well-indicated hyaluronic acid restores targeted support that redefines the contour. Always under medical indication: injecting volume where what is missing is tightening is a frequent — and expensive — mistake.

5The honest answer: surgery

Marked sagging

For significant descent with clear excess skin.

When there is excess skin, no non-invasive technology can remove it. If that is your case, we will tell you at the first consultation and point you in the right direction — a good aesthetic medicine professional knows when the patient is not theirs. That policy has earned us more trust than any treatment.

The double chin: the special case you ask us about most

The double chin deserves its own chapter because it has three distinct causes — and each is treated differently. It can be submental fat accumulation (common even in young, slim people, with a genetic component), tissue laxity after losing collagen or weight, or a combination of both. Identifying which is your case is exactly what the assessment does.

For a laxity-based or mixed double chin, HIFU is our star indication: focused ultrasound tightens the tissue and helps remodel submental fat, redefining the jawline without surgery, usually in a single yearly session. Radiofrequency complements it by improving the skin quality of the neck — an area that gives away age more than the face itself.

4 myths worth retiring

"A good firming cream will restore my jawline."

Creams hydrate and improve surface texture, but no cosmetic reaches the SMAS or tightens sagging tissue. They are maintenance, not treatment.

"Facial gymnastics and face yoga eliminate the double chin."

The evidence is very limited. In some cases, building up certain muscles can even accentuate folds. If there is fat or laxity, facial exercise does not correct them.

"Home radiofrequency gadgets do the same as professional ones."

They work at far lower power and depth for safety reasons. They may give a superficial flash effect, but they do not replace the energy or control of professional equipment applied with judgement.

"Sagging is just a matter of age; nothing can be done."

Age is one factor, but sun, tobacco, yo-yo dieting and genetics weigh as much or more. Almost all of those fronts can be acted upon — and the earlier, the less it takes.

In sagging, the most expensive mistake is treating the wrong plane: injecting volume where what is missing is tightening, or tightening where what is missing is support. That is why in our clinic the diagnosis is not a formality: it is the most important decision of the whole process.

Frequently asked questions about facial sagging

Can facial sagging be eliminated without surgery?

In mild and moderate degrees, it can be visibly improved: radiofrequency stimulates skin collagen and HIFU tightens the SMAS, the facelift plane. In marked sagging with clear excess skin, non-invasive technologies improve but do not match surgery — and we will tell you honestly if that is your case.

HIFU or radiofrequency: which is better for sagging?

They don't compete: they work at different depths. 448 kHz radiofrequency improves skin quality and firmness (mild laxity, maintenance); HIFU reaches the SMAS and is the reference for the lifting effect in moderate sagging, double chin and neck. In many cases the best plan combines both.

What really works for the double chin?

It depends on the cause. If it is laxity-based or mixed, HIFU is the reference indication: it tightens the tissue and helps remodel submental fat in one yearly session. If fat predominates, we evaluate the approach in consultation. Creams and massages do not eliminate an established double chin.

At what age should sagging be treated?

There is no fixed age: it depends on your skin, your genetics and your habits. Collagen loss starts gradually from your mid-late twenties, so as soon as you notice loss of firmness, radiofrequency and good habits slow the process with far less effort than correcting it years later.

Do these treatments hurt or require downtime?

Radiofrequency feels like a warm, pleasant massage. HIFU produces deep heat and small, tolerable energy pricks, without anaesthesia. In both cases you resume normal life immediately — at most, mild redness for a few hours.

How long do results last?

HIFU peaks at 3-6 months and lasts 12-18 months, with one yearly maintenance session. Radiofrequency builds results with protocols of 6-10 sessions and monthly maintenance. Habits — sunscreen, not smoking — multiply the duration of any treatment.

How do I know what degree of sagging I have?

This guide's orientation helps, but the reliable answer comes from an assessment: at our clinic we combine the facial diagnosis with artificial intelligence — which maps your skin zone by zone with objective data — with the medical examination of the jawline, neck and tissue quality. It is free and you leave with a clear plan.
